Requirements
- Individuals
- Completed application for Disabled Parking Permit signed by a physician or certified medical authority dated within the last year
- Application required for initial, renewal, or replacement permit and are valid for one year
- Florida drivers license or identification card
- Organizations providing regular transportation services to disabled persons
- Completed application for Disabled Parking Permit
- Florida sales tax or federal employer identification number (FEID) is required
Applicants may bring their application and payment (if applicable) in person to any tax collector office, drive-thru, or deposit it in any tax collector drop box. Locations can be found here.
Wheel Chair License Plates
- Application for International Wheel Chair Plate (Form HSMV 83007)
- Completed Disabled Parking Permit Application (Form HSMV 83039)
- Vehicle registration and proof of Florida insurance
- Florida drivers license or identification card

Permanent Permit – No cost
The blue disabled parking permit is valid for four years and expires on the owner’s birthday. Permits issued to an organization expire in four years on June 30.
Temporary Permit – $15.00
The red temporary permit is issued for a time period specified by the certifying authority, not to exceed six months. Temporary permits may be extended by completing a new application. If an additional permit is applied for within 12 months from the issuance of the first permit, no additional fee is required.
