Did you know that you can update the address on your Real ID-compliant driver license or identification card or replace a lost or stolen Real ID-compliant credential online? Click here to renew or replace online.

Please bring the following documents for issuance of a Real ID-compliant Florida Driver License or Identification Card. Be prepared to have your photo taken and your vision checked. If corrected lenses are required, please have them with you.

  • Certified U.S. birth certificate or valid U.S. passport in current name
    • If customer’s name has changed since birth, we need certified proof of legal name change(s) (certified marriage certificate, certified court order, etc). Documents must link birth record to current name.
  • Social Security card or acceptable proof of number
    • Social Security name change must be made at least 24 hours prior to visiting our office

  • Residential Address Requirements
    The residential address for a Class E Florida license or ID card must be a Florida address. Two proofs of residential address are required. Each proof of residential address document must come from a different source.
    • Copies of residential address documents and documents used to establish relationship for address requirements are acceptable.
    • Envelopes cannot be accepted as proof of residential address for a Class E driver license or ID card.
    • Proof of residential address can be accepted by e-mail if the document comes from the company or organization that issued the document and not from the customer.

      The following proofs of address are acceptable:
    • Household information document- Deed, mortgage, monthly mortgage statement, or residential rental/lease agreement
    • Florida Voter Registration Card
    • Valid Florida Vehicle/Vessel Registration or Title
    • Utilities bills – (Ex. water, gas, electricity, cell phone, cable) (must be dated within the last 30 days)
    • Automobile Payment Booklet (must be dated within the last 30 days)
    • Selective Service card (must be dated within the last 30 days)
    • Medical or Health card and/or Medical bill (must be dated within the last 30 days)
    • Homeowner’s and/or Automobile insurance policy or bill (must be dated within the last 30 days)
    • Employer Documentation (W-2 form or 1099 form, paycheck stub) (must be dated within the last 30 days)
    • A letter from a homeless shelter, transitional service provider, or half-way house verifying that the customer resides at the shelter address (must be dated within the last 30 days)
    • Financial Statements (bank, credit cards or investment accounts) (must be dated within the last 30 days)
    • Mail/Documents issued by Federal, State, County or City government agencies -including city and county agencies (must be dated within the last 30 days)
    • Sexual Offender/Sexual Predator/Career Offender: (must be dated within the last 30 days)
      Certification of Address, section A, and the FDLE Registration form completed by the local sheriff’s department.

Address Certification:

An applicant who resides with someone else, including a minor child residing with their parents, may provide proof of residency for that other person as well as a completed Certification of Address Form

The certifier’s signature must be notarized or witnessed by issuance personnel when the customer is using that person’s residential address documents. This form may also be completed by visiting our office with the applicant and the person with whom they reside and who is providing proof of residency.

Examples:

Acceptable – utility bills from two different utilities

Not-Acceptable – two utility bills from the same utility

Acceptable – Florida vehicle registration and Florida Voter Registration Card

Not-Acceptable – two documents from the Florida Department of Highway Safety and Motor Vehicles

Military members and veterans should present a copy of their DD-214, which shows their honorable discharge from active duty, to receive a veteran designation.

Please be aware the identity documents must be certified originals, photocopies of the documents cannot be acceptedHospital birth certificates are not considered a certified document and will not be accepted.

During your transaction, you will be asked to provide emergency contact information that will allow law enforcement authorities nationwide to contact designated family or friends if an emergency arises. If you plan on making application for voter registration when you get your license, you should know that Florida is a closed primary state. Only voters who are registered members of political parties may vote for respective party candidates or nominees for an office in a primary election, including a presidential preference primary election. Any voter application with party designation or party change must be done 29 days before an election.

Note: REAL ID-compliant cards have a gold star on them. If you are changing the name on a REAL ID compliant card, you will need to provide name change documents (e.g. marriage certificate, divorce decree). If you are visiting an office for a driver license or identification card transaction, bring proof of address with you even if you have not moved since your last transaction.
