Title Request Form for New Residents and Military Personnel

Request a Title Held by a Lienholder

Use this form if you are transferring your title and registration to Florida and the title is held by a Lienholder. Please only use this form if you have a residential address in Escambia County, Florida.
This choice will determine how your application is tracked and which documents and instructions are provided to you.

Vehicle Information

Enter your Vehicle Identification Number (VIN). Usually 17 characters long, but could be as few as 9 characters depending on the age of the vehicle. Your VIN can usually be located on your current registration, loan documents, bill of sale, dashboard of your vehicle, or title.
Current Titled State(Required)
Indicate state where vehicle is currently titled. The following states allow the owner to hold the title even if there is a lien: KY, MD, MI OPTIONAL, MN, MO, MT, NJ, NY, OK, WI, WY OPTIONAL This means that if you are from one of these states, you most likely possess your title.
Status of Current Registration
MM slash DD slash YYYY
Example: Chevrolet, GMC, Ford, Honda, Toyota, Kia, Hyundai, etc.
Example: Camry, Explorer, Altima, Silverado, etc.
A “pickup” is defined as a truck weighing less than 5,000 pounds when unburdened. If your truck weighs more than 5,000 pounds, choose “truck” from the list below. If you are unsure, make your best guess. This information can be corrected later during the titling process.
Choose your lienholder from the list below. A lienholder is usually a bank or financial institution who holds an interest in your vehicle and to whom you are making payments.
If your lienholder was not listed, type the name of the bank, financial institution, or individual below.
The loan number is helpful in ensuring that your lienholder locates the correct vehicle record. Your loan number may be located on your billing statement, original loan documents from time of purchase, or by contacting your lienholder.

Owner/Lessee/Lessor Information

Below, you will enter the name(s) of the titled owners. If your vehicle is leased, the owner will usually be the leasing company. Lessor: Company or individual who owns and leases the vehicle. Lessee: Individual in possession of the vehicle and usually the one making payments to the lessor for use of the vehicle.

Individual who leases the vehicle from a lessor.

If someone owns or leases the vehicle with you, list their information here. Their signature and identity documents will be needed during the titling process.

If you have additional comments or a unique situation, please describe it here. You may wish to indicate if you have had a previous license plate history in Florida or other pertinent information. You can indicate if you wish to complete the transaction by mail. Please do not include personal details or payment information in this field.
