Military Orders showing Florida as your duty station (submit a copy)

For Florida resident military personnel stationed outside of Florida pursuant to military orders, you will need:

Military Insurance Affidavit PDF (available through the portal below or found here)
Copy of Insurance Card (may be a state other than Florida)
Military Orders (submit a copy)
